I am a professional seeking supervision and/or mentoring. I understand that this is not a clinical service.

I understand and agree that the supervision fee that I pay is non-refundable if I cancel, do not attend my session, or reschedule with less than 48 hours' notice. If my session has to be cancelled by Protea Therapy, or if I contact Protea Therapy to reschedule with more than 48 hours' notice, I will be offered the opportunity to reschedule.
